MEDICAL ALERT
One of the key parts of the athlete physical is the back of the paper. The athlete signature gives us permission to: 1)take your picture and use it for publicity; 2) give out your medical information to a hospital or emergency personal; 3) gives Special Olympics the authority to act on your behalf.
If you have a change of address or change in your phone number or your emergency contact person has changed -- Please update your physical or tell your coach.
